  • This is a full time, remote position virtually supporting our Connecticut-based patients. Join our primary care team in delivering telehealth urgent care services as part of a patient-centered medical home.

As Telehealth Urgent Care Provider for Community Health Center, you will:
  • Assess and treat common acute and episodic illness in adults and children
  • Perform/prescribe/order appropriate diagnostic testing as required and arrange for follow-up care with PCP as indicated
  • Recommend preventative services as appropriate
  • Provide problem-specific education to individual and family members as appropriate
  • Utilize evidence based standards in formulating treatment plan, and communicate rationale for decision-making with patients
  • Document all findings, including appropriate plan for follow up care, in electronic health record
  • Participate in the organization's quality improvement program
  • Effectively collaborate with other CHC providers and Clinical Operations staff
  • Stay abreast of current trends in healthcare practice and participate in continuing medical education to ensure optimal clinical knowledge and effective patient care skill.
  • APRN/PA Qualifications - Required for this role:
    • Graduate of an accredited Master of Science in Nursing program or Physician Assistant Master's program
    • Licensed in Connecticut as either a Family Nurse Practitioner or Physician Assistant
    • Must be an expert user of telehealth and other electronic platforms necessary to complete visits, and care coordination associated with patient care in this model.Must have access to reliable internet connectivity and suitable home office environment, and be competent with technology to ensure a positive patient experience
    • Bi-lingual (Spanish) language skills preferred but not required
