Our client is currently seeking a Patient Care Coordinators to work in Irwin, PA

This job will have the following responsibilities:
- Greet patients
- Collect patient payments
- Collect and assist patients with completing paperwork
- Answer patients' questions in person and on phone
- Review and support implementation of patient plan of care
- Coordinate services/case management for non-episodic and non-catastrophic cases
- Explain and talk to patients or caregivers about sensitive health care decisions such as advanced directives, physician's order for life sustaining treatment , and palliative care/hospice referrals
- High School Diploma or equivalency (GED).
- 1+ years of customer service OR experience in a medical office setting.
- Proficiency with basic computer software.
- Willingness to learn electronic medical records systems.
