Overview

The Nurse Liaison (Warm Springs) is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality, safe patient care. It is expected that all RN Clinical Nurses - are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of Practice and Code of Ethics Standards put forth by the American Nurses Association.

Job Details

Facility: Wellstar West Georgia Medical Center

This position is a full-time role located at the Roosevelt Warm Springs location.

Hours - 8:00am - 5:00pm Monday-Friday

Job Summary:

Responsible for assisting with the coordination of the daily operations for all preadmission activities, completing eRehab and LTRAX assessments, participating in the planning, organizing, and executing of the business development plan, and developing marketing strategies through social media to expand the general public's knowledge of the facility. Essential responsibilities accomplished by working closely with the Nurse Navigators to convert referrals to admissions by providing responsive customer service and serving to promote RWSH.

Core Responsibilities and Essential Functions:

  • Provides Marketing support by understanding healthcare operations, trends, and data analytics to promote and establish contact with potential customers or referral sources.
  • Creates and provides education materials for referral sources, physicians on services, programs, and outcomes.
  • Delivers content for social media accounts.
  • Assists DBD with coordination of Admissions with the following tasks, but not limited to: maintaining tracking files, completing report emails, leading morning conference call, assisting Nurse Navigators and Admission Liaisons, providing facility tours, providing field coverage, collaborating with hospital directors and maintaining employee and patient confidentiality.
  • Completes clinical evaluations by discussing with potential patients and families of pre-admission and admission processes.
  • Accurately performs assessments and completes pre-admission screens in a timely manner.
  • Facilitates and coordinates with physician and DBD as necessary for admission.
  • Maintains compliance with Patient/Family-Centered care standards by providing information sharing, ensuring patients and families receive timely, complete and accurate information in order to effectively participate in care and decision-making.
  • Maintains compliance with RWSH Customer Service standards.
  • Other duties as assigned.

Required Minimum Education:

Vocational or technical training or degree: Licensed Practical Nurse. Required.

Required Minimum License(s) and Certification(s):

Current American Heart Association, Basic Life Support. Required

Current Licensed to practice as a nurse (LPN) in the State of Georgia or have an active Multi-State LPN Compact License. Required

Valid GA Driver's License. Required

Required Minimum Experience:

2 year(s) or more of experience in direct patient care as an LPN. Required

1 year(s) or more of experience in navigating patients through the continuum of their care. Preferred

Required Minimum Skills:

Professional, organized, problem solving/critical thinking, communication skills - verbal and written. Required
Ability to interact appropriately with patients from various backgrounds. Required
Must be able to build and sustain trust with the physician, Clinical Manager and the interdisciplinary care team. Required
