About the Department

The Pediatric and Adolescent HIV/AIDS Program, Care2Prevent, in the section of Pediatric Infectious Diseases works under the direction of the Principal Investigator and Clinical Director to provide patient care, case management, behavioral health services, and prevention and outreach programs for people living with HIV and those who are most vulnerable to HIV infection. The Medical Case Coordinator monitors a caseload of up to 35-40 patients who are living with HIV. The Medical Case Coordinator conducts initial assessments/reassessments of patient needs, implements service plans, identifies gaps in services, and provides ongoing support. Job Summary

The Medical Case Coordinator advocates for patients/clients, identifies obstacles to maintaining ongoing engagement in HIV care, and maintains appropriate documentation of client interactions and services provided. The Medical Case Coordinator supports outpatient medical clinics to ensure eligible patients receive case management services and helps to monitor the weekly clinic schedule to ensure appointment adherence. The Medical Case Coordinator attends interdisciplinary team meetings, mandatory trainings, case conferences, participates in pager coverage, and community outreach/education. Upon completion of training, the Medical Case Coordinator is expected to obtain certification in HIV case management from the AIDS Foundation of Chicago within 90 days of employment.

This position is wholly funded by contractual grant funding which is renewed under provisions set by the grantor of the contract. Employment will be contingent upon the continued receipt of these grant funds and satisfactory job performance.

Responsibilities

  • Provides patient-centered resource counseling and advocacy to Care2Prevent clients. Conducts initial assessments/reassessments of basic needs, psychosocial needs, medical needs, and identifies linkages and referrals as appropriate to client needs.
  • Provides one-on-one support, referrals to psychosocial support services, addresses health insurance coverage needs, assists with and/or arranges transportation, food and housing, and other resources as needed.
  • Maintains client privacy and confidentially in compliance with HIPAA guidelines as well as policies outlined by the Care2Prevent program, the university, and funding agencies.
  • Maintains detailed client files including case notes, referral requests, and other reporting documentation as required by the Care2Prevent program, the university, and funding agencies.
  • Maintains regular contact with clients to support adherence to care and performs home visits as appropriate (up to 20% local travel).
  • Occasional evening hours are required.
  • Screens and assesses individuals and/or families to determine education and counseling needs. Counsels individuals and/or families on the basic principles of health education.
  • Supports individual and community health by contributing to the building and maintaining working relationships with those in public health care. Serves as an educational resource and patient advocate.
  • Performs other related work as needed.

Preferred Qualifications

Education:
  • Bachelor's degree.
  • Master's degree in a human services field.

Experience:
  • Lived experience in communities of focus is a plus.
  • 1-2 years of related social service experience strongly preferred.

Preferred Competencies
  • Sensitivity to and understanding of issues concerning HIV.
  • Experience working with diverse populations including that of minorities and LGBTQIA+.
  • Familiarity with group facilitation and/or willingness to learn.
  • Detail-oriented with the ability to act as a liaison between clients and service providers.
  • Microsoft Office experience and general computer experience.
  • Ability to adjust well under high stress and ever changing environments.
  • Attention to detail and ability to prioritize independently.
  • Ability to maintain confidentiality, problem-solve, reason and communicate effectively in both verbal and written form.
  • Comfortable with frequent communication via phone and e-mail.
  • High level of proficiency with time management.
