Minimum Data Set (MDS) Coordinator

Maplewood Care Center is looking for an experienced MDS Nurse/Coordinator to join its team. The MDS Coordinator is responsible for the Minimum Data Set (MDS) Comprehensive Resident Assessment Instrument of the facility residents.

Objective: Our MDS coordinators are critical to evaluating and managing cares, as well as maintaining accurate medical records and coding procedures in order to ensure proper reimbursement. In addition to data sets you will also be an intricate part of advocating for residents' rights and quality care.

Core Responsibilities:

  • Provides Leadership and Direction on Assigned Units with all staff.
  • Resident Care Functions which include medication, treatments and follow nursing protocols.
  • Immediately reports any suspected abuse or neglect per company policy.
  • Charting, Documentation and Floor Functions.
  • Hands on, medication passes, and conducting assessments.
Schedule: 8:00a-4:30p, Part-time

Pay: $35.02-$35.72 per hour depending upon experience

CORE RESPONSIBILITIES:
  • Coordinating the completion of the minimum data set assessment (MDS) and Care Area Assessment (CAAS).
  • Ensure that care plans accurately reflect the cares and clinical monitoring provided.
  • Monitors regulatory compliance for completion of MDS/RAI process
  • Informs IDT members of validation errors as appropriate.
  • Follows the HCFA Correction Policy for corrections.
  • Stay abreast to changing MDS/RAI process regulations.

QUALIFICATIONS:
  • Current licensure in the state of employment as a Registered Nurse.
  • Previous RN long- term care experience with working knowledge of the RAI process preferred.
  • Must have a RAC-CT Certification or other accredited MDS Program.
  • Good problem solver with abilities to make independent judgments utilizing initiative, discretion and the nursing process as needed.
  • Demonstrates good organizational skills.
  • Displays knowledge of RAI/PPS process.
  • Ability to prioritize duties, and work independently.
  • Customer service oriented with knowledge of successful customer service techniques.
  • Ability to complete assigned duties within a constantly changing environment.
  • Team player with ability to function within interdisciplinary team.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
  • Possesses knowledge and understanding of rehabilitative nursing concepts.
  • Clinically competent to utilize the nursing process to perform physical and cognitive assessments of residents.
  • Establish and maintain optimum rapport with staff, residents, family members and regulatory agencies.
  • Ability to utilize computers and other electronic devices for tasks
  • Resident Assessment Coordinator Certified Program, RAC-CT needs to be certified six (6) months after hire.
