The Licensed Clinical Social Worker - GME Internal Medicine @ AUSTELL is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality, safe patient care. It is expected that all RN Clinical Nurses - are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of Practice and Code of Ethics Standards put forth by the American Nurses Association.

Job Summary:

The Social Worker Advanced Practice (LCSW) functions as a licensed clinical social worker. In this role, (s)he is primarily responsible for guiding patients from a social work aspect identifying and connecting them with resources for basic living needs such as food, shelter, transportation, medical insurance resources, in coordination with other members of the medical team.

Core Responsibilities and Essential Functions:

Evaluates new and established patients through Social Determinants of Health evaluation. Assists patient in mobilizing support system. Completes documentation for all patient conversations- Assists patients in completion of short term and long term disability paperwork - Documents social work conversations using progress notes - Documents prioritized quality and value metrics, as determined by the WellStar Medical Group - Coordinates informational meetings and phone consultations with Medicaid/ Department of Community Health and facilities in the community for collaboration and continuity of care - Provides community resources: Transportation services - Housing resources such as shelters, public housing, rent and utility assistance resources- Food pantry resources - Medication assistance resources and applications - Medicaid, SNAP, TANF application assistance - Wellstar Financial Assistance application support - Support group referrals - Domestic violence resources - Substance abuse and recovery resources - Eating disorder education and referrals - Student resources, disability services - ECT service referrals.

Required Minimum Education:

Master's Degree in Social Work from an accredited School of social work. Required

Required Minimum License(s) and Certification(s):

Licensed Clinical Social Worker GA 1.00 Required

Additional Licenses and Certifications

Required Minimum Experience:

Minimum 3 years experience in a health related field, mental health, behavioral health, counseling or social services agency. Required

Required Minimum Skills:

Crisis Intervention, treatment evaluation, community collaboration, knowledge of treatment resources, knowledge of disability services, etc.
Possesses emotional maturity.
Demonstrates ability to work with patients with a life limiting illness.
Demonstrates ability to work as a member of an interdisciplinary team.
Possesses psychotherapy/counseling skills that can be applied to individuals, families, and groups.
Possesses knowledge of developmental characteristics (physical, psychosocial, intellectual) of people of all ages, economic backgrounds and cultural backgrounds.
Understands the documentation requirements of behavioral health providers to ensure protection of privileged communications shared in the context of psychological treatments. Demonstrate basic use of computer: Windows, Outlook and Microsoft Office.
