Merakey is seeking a Full-Time Dual Diagnosis (Mental Health and Developmental Disability) Case Manager based out of Sandusky, Ohio serving counties across North Central Ohio. The Dual Diagnosis Treatment Team (DDTT) program offers support to children and adults who have both a serious mental illness and a developmental disability. Participants receive counseling, physical healthcare, support finding housing, assistance during crises, and help dealing with other issues in their daily lives.

The Case Manager will be responsible for:

• collaborating with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan.

• performing initial intakes, assessments, and timeline summaries.

• providing intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.

• providing individual and group treatment and support according to each individual's/family's needs.

• supporting data collection, monitoring interventions, and providing feedback to the treatment team regarding client progress.

Required:

• Bachelor's degree in social work, psychology, or another human services related field

• Three years' experience in a mental health and/or developmental disability setting

Preferred:

• at least one year of experience in a mental health setting and one year working in a developmental disability setting
