Interested in a hybrid position? The Judge Group is seeking for a Case Manager position in Columbus, OH!

For this hybrid position the desired schedule is 8AM -5PM with flexibility. Willing to commute 50-100 miles per day, if needed.

This job will have the following responsibilities:

• Assists members with housing, food, clothing, heating, transportation, scheduling appointments, medication refills, obtaining DME, maintaining eligibility, and obtaining resources to assist with financial assistance.
  
• Assists the member in navigating their healthcare needs and connecting them to community-based resources, education, advocacy and social support.
  
• Provides outreach to locate and/or provide support for disconnected members with special needs.
  
• Participates in ongoing or project-based activities that may require extensive member outreach (telephonically and/or face-to-face).
  
• Scheduling appointments with providers.
  
• Arranging transportation for healthcare visits.
  
• Getting prescriptions filled and following up with members on missed appointments.
  

Qualifications & Requirements:

• HS Diploma/GED
  
• Minimum 1 year experience working with underserved or special needs populations.
  
• Must be an LPN, LSW or CHW in good standing with the State of Ohio.
  
• Must have valid driver's license with good driving record and be able to drive within applicable state or locality with reliable transportation.
